Erros diagnósticos da síndrome da obesidade- -hipoventilação no Departamento de Emergência: uma série de casos

Objetivo: Relatar casos de pacientes com diagnóstico posterior de síndrome da obesidade-hipoventilação e, retrospectivamente, as situações dos equívocos diagnósticos relacionados ao quadro nos ambientes de pronto-atendimento. Métodos: Estudo observacional, descritivo, exploratório e retrospectivo em prontuários de pacientes que procuraram Serviços de Emergência públicos e privados. Resultados: Foram avaliados 11 diagnósticos de síndrome da obesidade-hipoventilação, após atendimento de emergência. Pacientes apresentavam índice de massa corporal médio de 46.2 kg/m². Em 90.9% dos casos, não houve diagnóstico de síndrome da obesidade-hipoventilação no pronto-atendimento. Os principais motivos de procura do pronto atendimento foram infecções respiratórias e descompensação cardíaca. Todos os pacientes apresentavam sintomas de apneia obstrutiva do sono, e 90,9% tiveram diagnóstico equivocado de doença pulmonar obstrutiva crônica. Houve um óbito, e documentou-se tempo de internação prolongado. Conclusão: Destaca-se a compreensão limitada da síndrome da obesidade-hipoventilação por parte das unidades de urgência, o que resulta em dificuldades de obterem diagnósticos precisos e, consequentemente, o tratamento adequado.
